Russia: Worker killed at VSMPO-Avisma plant due to safety violations, co. fined

Rostekhnadzor completed the investigation of the accident at PJSC VSMPO-AVISMA Corporation, 19 August 2020

On July 22, 2020, the Ural Department of the Federal Service for Environmental, Technological and Nuclear Supervision (Rostechnadzor) completed an investigation into a fatal accident that occurred at a hazardous production facility of the Public Joint Stock Company VSMPO-AVISMA Corporation .

On May 30, 2020, on the instructions of the workshop shift supervisor, a team of slingers, using a railway crane, unloaded scrap metal from a gondola car. At 14:46, while attempting to lift a large cylindrical part, a slinger inside the gondola car was fatally injured by a moving electromagnetic washer hung on the hook suspension of a railway crane.

The commission found that the main cause of the accident was the unsatisfactory organization of work, which was reflected in the improper implementation of production control over compliance with industrial safety requirements during the operation of lifting structures.

In addition, the investigation revealed unsatisfactory implementation of production control, expressed in the failure of specialists to strictly comply with the requirements of the Federal norms and rules in the field of industrial safety “Safety Rules for Hazardous Production Facilities Where Lifting Facilities are Used”, job descriptions, and by personnel - production instructions.

Based on the results of the investigation, VSMPO-AVISMA Corporation PJSC was held accountable for an administrative offense under Part 3 of Art. 9.1 of the Code of Administrative Offenses of the Russian Federation, with the imposition of a fine in the amount of 500,000 rubles. The materials were transferred to the investigative authorities.
