According to the police, the FIR was registered against the sub-contractors of Navayuga Engineering Company Limited and VSL Private Limited, the agencies hired to construct Package 16 of Samruddhi Expressway, the 701-km highway between Mumbai and Nagpur.

According to the FIR registered in Shahapur police station based on the complaint of Premprakash Ayodha, an injured worker, while launching the girder, it fell along with the gantry machine on the workers, engineers and supervisors at the site, and several died and a few workers sustained injuries.
